Applications

Families may apply for supplemental services through the foundation. Applications will be processed and decisions made each yearly quarter. The foundation will examine each request on a case-by-case basis.

Current governmental or waiver assistance is not a factor in receiving assistance from the SAT Foundation. We analyze the need and fill it when possible.

Funds will be released to the service provider.

Applicants must meet the following criteria:

  • Adults with an intellectual disability in a home health environment supported by aging parents or primary caregivers
  • Adults with an intellectual disability who are referred by a social worker, support broker or home health personnel
  • Adults with an intellectual disability who are on an ephemeral wait list period for benefits
  • Adults with an intellectual disability who have been denied benefits
